When you check out of the office after your appointment today, we will make a copy of your written prescription for the test and a copy of your insurance card to sent to University Diagnostic Institution (UDI). UDI will be scheduling your test or procedure and will contact you within 2 days of your appointment in order to schedule your test or procedure.
We will give you a copy of the information sheet from UDI that includes their phone number and directions to their facilities. If UDI has not contacted you within 2 days, please call them at 407-975-3315.
When you go to your appointment for the test, you must bring your physician’s or physician assistant’s written prescription and your insurance card.
It usually takes one week after your procedure for the test results to reach our office. Please call our office if we have not contacted you within one week of completing your test.
If you have made your follow-up appointment before you have completed your tests, please give yourself enough time so that all the results and films will be here for that appointment. If you have not yet made your follow-up appointment, please call our office to schedule that appointment.
UDI will send the tests directly to our office. If you use another facility besides UDI, please make sure that you pickup your films and reports from your test to bring with you to your follow-up appointment.
If we do not have the films at the time of your appointment, we will need to re-schedule your appointment.
